This might be a bit obscure - think of the numbers 0 - 15. The actual size is 55mm x 8mm diam. We were issued with them but I never knew anybody who actually used one.

Correct! On the back of IBM SCSI devices was a rotary dip switch to set the channel number. You could turn it with a pair of pliers, which is what we used.

It is indeed a mining array. This one is based on usb dongles. I'm out of the office ATM but will post up pics of the whole thing tomorrow. The house based arrays are very impressive too.
