So Lincolnshire DNS uses OpenDNS for upstream queries? That's not exactly playing by the rules. OpenDNS will probably ban the forwarders as soon as they notice the traffic volumes. -
That configuration section is not relevant to mod_userdir configuration. Please post the correct section.

The apps are installed in the /system partition on the phone, that's why you can't remove them. If you root the phone you can delete things in /system. Alternatively just disable/hide them (assuming you have Android 4.x). Ultimately the best way to get rid of the bundled crud that comes with an android phone is to install a custom rom that only has the things you want. There's plenty of 'minimal' style roms for most make/model of Android phone.

I think you can DIY this with FreePBX. You basically want some extensions setup that go to Phones/Computers/etc which are setup to Auto Answer (or know what paging is) and Speaker Phone on (or connected up to some speakers for a Computer). You can certainly do this with an old laptop running Xlite3 for example. If you want more complex setups then you could make ringgroups for the extensions, so you could call groups of rooms together. Protip: Use an analog joystick that doesn't auto centre. You'll lose a lot less paintwork docking at Lave that way.

I don't think Eve is comparable. One of the most fun things about Elite was flying the ship yourself in real time. Something Eve doesn't let you do.

Use a carrot and stick approach. Give them a discount (say 50p?) if they replace straight away and therefore don't require a temporary card. If they use the temporary card and don't replace it then disable the temporary card after a set time period and optionally automatically bill them from their credit for a new permanent card. Always give them the option of paying from their credit, the easier it is for them to get a new card then the more likely it is they will do it.

Yeah it's in everyone's best interest if these get removed. So you don't get hits on your filtering and so they don't have a PR problem.
