We have them turned on along with Spamhaus filtering. We also have more powerful upstream filtering provided by our LEA and other agencies. Either way, it's a pretty good setup - some junk gets through but not much. Getting SPF set up for our mail domain has also made a huge difference.

Yes you can I had them on in my last job and all mail went through SmoothZap before Exchange as IMHO the built in Exchange ones were not much cop basic stuff was still getting through so you use it and something else before it and that seemed to work pretty well.
