Take a look at the web filtering, I have seen a few autodiscover addresses being blocked that causes this, mainly with Sophos UTM.

+1 this - our autodiscover address for o365 was filtered out of the box on our lightspeed solution!

had this years ago. if i recall it had something to do with outlook anywhere. can be disabled in the account setting in outlook or set via Group policy.

I had this because an update wasn't installed. When I get to a computer I will post what update it was.
