I use Dovestones as well easy to use and for the cost you get a bunch of tools to make your life easier plus their support is friendly and quick I've even had replies at 11pm at night from them.

similar to sysman_mk ive got a spreadsheet i use looks a bit simpler but works just dump first name / last name / year of entry in and it generates usernames (though dosent check for duplicates and can throw a paddy if it finds a space) then once ive filled in the details i just copy and paste form the last 2 sheets into a batch file and run on server

Have used Dovestones AD Bulk User previously as well.

 

Managed to setup a mdb that pulled from SIMS SQL, manipulated the data and chucked out a csv file which dovestones then imported to AD using the command line features of AD Bulk User, created home directories as well using scheduled tasks. Took a while to setup as I'm no access expert. But once in place with and with a standardised password format once student on SIMS the following day they could log into AD without even coming to the IT office to get a user account setup :-) .
