So do you buy them outright or are they on some kind of horrid yearly licence thing with the potential for yearly licence and support costs to increase drastically over a 10 year lifetime?

The purchase would be outright. Unless you opted to go down a lease/finance route. You get lifetime warranty on all off their switches. You do have the option to go above and beyond the warranty with 8x5, or 24x7 technical support - this is included for the first 90 days of purchase.
