Would it be a good idea to create a question type and see if that works? How would I do that?

Here it is although I dont think it will help you much. Basically, you click on edit quiz but the list of question types does not appear

 

From your screen shot the moodle page footer is missing which would mean the page has failed to load correctly.

Turn debugging on to developer and tick the display error message box underneath and see if the page shows an error.

If you host on IIS have a look at c:\windows\temp\php54_error.log or c:\windows\temp\php53_error.log and it should list a PHP Fatal Error message. That would help define what is causing it.

1)Were you migrated directly from 1.9 to 2.2? There are warnings in the migration notes about the quiz question engine changing and to be aware of this - Upgrading to Moodle 2.1 - MoodleDocs

2) As mentioned on the Moodle,org forum, have you tried with a different theme to make sure it's not your theme?

3) Can you go to Site administration>Plugins>Question types>Manage question types and do a screenshot of the available question types -I will tell you if there are non-standard ones there that might be conflicting with your 2.2 install.

We have tried lots of themes with no success

We migrated from 1.9 to 2.0 and then immediately up to 2.2

Your article seems to mention issues with quiz data rather than quiz questions?

I cannot give you a screenshot as there is absolutely nothing in that list...

there is absolutely nothing in that list... Er - there should be a list of all the question types (here is a screenshot from 2.4 as I don't have an earlier version but if you don't see anything then we need to investigate further...)

If your page is blank, you can not create a question...that is your real problem

I see you have a few responses since earlier today. Almost bringing you back full circle.

 

I tried to re-read all your postings to catch up and refresh my memory. But I do not recall seeing..MOODLE version # and if you did an upgrade recently? Or, can you confirm that it worked at least once prior and suddenly stopped or what happened, when or how did this problem appear or occur?

We upgraded to 2.2 at Christmas.The IT coordinator thinks he has created a quiz since then but he is not sure

When I say there is nothing in that list, what I meant is that it doesn't display, I get an "HTTP 500 internal server error"

Other things, such as "Manage Question Behaviors" works fine

 

*nb there is a note that the Quiz Question Engine has been updated successfully so I dont think that is the issue

I can't find these. There are no error.logs in the temp folder and nothing that starts php54 either

Can't find php.ini either - I am very confused now. Am I looking on the local pc or the server - because there is nothing in either location. I dont have debugging turned on now but I did yesterday so error logs should have been created then?

Try again with looking at the Manage questions page in Moodle. From Site administration>Development>debugging, set it to "developer" and then make sure you have ticked "Display debug messages" Then when you try to open the Manage questions page it should display some code you can copy and paste

This is what I get:

Which would back up my idea that the questions and question types just aren't there - what on earth can I do about that?

Ok well that solves it I think - it's looking for non-standard question types - gapselect and ddimageortext from the OU. You need to go in and get rid of their traces or I guess maybe add them and try again Edited October 9, 2013 by secretlife

Assuming you have access to your Moodle on the server? Go to your Moodle folder and find the "question" folder. In there look for the "type" directory and delete the two that say "gapselect" and "ddimagortext" Then go in to your Moodle as admin and click on Site administration>Noficiations to see if it recognises the plugins have gone -then go back to the Manage question page and if they are there, delete them

I would install the missing plugins as it looks like you have question already created with them

I agree with Penfold - populate the (moodle)/question/type folder from the Moodle.org download section. It sounds like you're missing the whole folder though, standard questions included.

Now, I admit, I have passed this problem on to mrwITch as he knows far more about this stuff than I do. Firstly he installed gapselect as it said it was missing within dd image or text but that didn't make a difference. After reading your posts he thought perhaps he should bring down the latest version of dd image or text as maybe it was a conflict between an old and new version. That seemed ok but it threw up another problem with another bit, which he upgraded, and so on until the errors disappeared

It seems to be much better now and the standard questions seem to be there. I will wait until the ICT coord tells me if all is well on Monday

Good! - It will be good to know on Monday if it does work for future reference advising people - as this issue (with non-standard question types not being upgraded corrupting the questions page) seems to have come up before. Edited October 11, 2013 by secretlife
