iPads issued to Staff - form to sign?

Does anyone have a copy of a form that they ask Staff to sign when issuing iPads? We had an old one for laptops that stated:

Issued to ___________

Department__________

Date out__________

Date returned________

 

and that was it. I am trying to create something with more details. We have iPad Minis to assign to members of SLT/SLC, some will look after them well, others I can see leave them lying around.

This is what we use, with changes made for long term loan, equipment type and such like and it appears to cover most eventualities.
