PDF Sanity check needed please

Could someone please check if they can edit the form fields in this PDF and:

 

a) have the form fields not go blank when they lose focus

b) print the contents of the form?

It WorksForMe ™ on Linux and OS X, but on Reader 10.1 on Windows it doesn't - almost as if the layering order on the form is wrong.

Works fine in 9 Not got 10.1 installed here.

 

I know at home with 10.1 sometimes on windows you'll need to allow interactive activity from set domains, but if you're downloading it not printing online? It should be ok.

Works fine for me in Adobe Reader 10 and Google Chrome's built-in PDF viewer.

Works fine for me on Adobe Reader 10. Filled out, saved, printed ... oh look I got 100/100 too!!!

It turns out if you open these editable PDFs in Preview on OS X, save it, then attempt to open it in Reader 10.1 (or earlier) on XP or 7, it breaks the form fields - the data is there, but doesn't appear unless you select the field and doesn't print.

 

So if your staff are filling in cover sheets, make sure they stick to one OS and don't pass them back-and-form cross-platform.
