ICT Evaluation and Staff Survey

I am looking to create a survey for the school staff soon to survey the IT Support department. I think its about time I establish a benchmark on how we are doing through the eyes of the people we support.

 

I was wondering has anyone done this and if so what type of questions have you asked?

Basicaly ask the questions that you need feedback on!

I have some documents somewhere on this I think - I'll try and dig them out.
