iPad agreement form for child/parents

Hi,

we have to supply an iPad to a child with a disability which the child will be able to use at home as well as at school. I can lock the iPad down with Meraki so the technical stuff is ok.

Does anyone have an agreement between parent & school form that I could use and personalise for the parent to sign accepting responsibility for the device?

yes we came up with one in the end, borrowed bits of someone else's etc.
