Broken share permissions

Hi, We are having a small issue where users write a file and occasionally it doesn't write any permissions to the file in the share. Does anyone know what can cause this problem ? Its a 2008r2 server with the end user successfully authenticated and mounted the network share.

 

I have attached a screenshot of a .doc file that was saved by a user and has no permissions assigned to it.

What do permissions on the folder that the file is in look like?

I wonder if we have a folder with no inheritable permissions...
