Anybody know a web-based media server we can host ourselves (Linux is fine) where a user can upload their video to the site, the server will automatically convert it to FLV, and the link can be shared with any other internal user who can stream the video through the embedded player?

No help but a suggestion, I'd pick h264 so smartphones and html5 stuff can do it, that way your content is not limited to just desktops. I think @CyberNerd was using ffmpeg and a monitored folder to re-encode stuff internally, not exactly the solution you are after but possibly a start.

We've setup a system doing this using a LAMP setup and Drupal 7 with the Video module, we convert to both H264 and FLV as a fall back. It's fairly straight forward to setup, you may have to compile ffmpeg on the server to get it to work properly though (we did).

Staff can upload a video in pretty much any format and it is then re-encoded with a cron task which we have running every 20 minutes to check for new content. It is possible to configure it to transcode on the fly but we found it took a while and staff just clicked submit again while they were waiting

There's also Octopus Video, which we're poking around with. Uses the Drupal Video module.

It is alpha code however, so depends on your comfort level.

That and Mediacore Community Edition.

Thanks for all the suggestions, going to give these a try.

For Octopus, right at the end of setup it said I must sign up for Zencoder and have an Amazon cloud account. It wouldn't let me self-host and just get past that screen. I don't even want those accounts. Any way to bypass?
