Hi Everyone,

 

Just wondered if anyone has come up with a telephone call log system using Microsoft Forms,

 

At the moment we are using an old Intranet for this and we are looking to come away from it,

 

We are looking to create a Forms template which then can be emailed to the relevant staff member with a copy of the phone conversation

 

I have looked at this using Powerautomate but i can't seem to see an option to forward the response to the relevant staff member based of a field in forms?

Happy to help - we have something similar.

 

One way to do it (i'm sure there are more better ways to, but hey this works)

 

As you said, you will need a field on the form of who you want the form sent to. You could call the field something along the lines of "for the attention of" where you admin/reception/contact centre staff will type in the email address of the releveant staff member. You could actually populate this further by having a list of the staff you can select, but if you're a small school/college typing the address should be fine.

 

Then over to power automate -

 

You need to add the "Send an email (v2)" action, and in the "to" field, you will put the response ID of the field on the form.

Pretty basic, but below:

Each branch has a different email body depending on the results of "Department" and gets sent to appropriate HOD.

 

They could be a fancier way of doing it, but works for me.

 

I do it this way, depending on the Department chosen there is a branch with a "hard-coded" email address.
