Microsoft Forms - Printed Form

Hi all,

 

Looking at creating a registration form for the pre-school of one of our primaries but one thing I would like it to do is generate a form that could be filed (either electronically, or printed and physically) against a childs record.

 

I know forms has a print button, but the output is rubbish. Anyone have any fancy (free!) ways of making the output a bit nicer. Have seen a couple of possibilities using Power Automate but both cost.
