Class Booking with 365

Afternoon,

 

I need to create a form or some kind of booking system so that parents can book their children into extra classes but with limited places.

 

Just wondering if anyone has managed to do this and how they done it?

I have looked at bookings but we only have Office 365 A1 Plus licensing so its not included, and we do not have money to get anything new.
