help with exam write pad (very new user)

Two questions on write pad.

 

1. How do I get the students details to come up so they fill these in before they start the exam. At the moment the box only opens when I press print.

 

2. in the written bit in the main file (Examwritepad v2.pdf) it shows a second tool bar at the top of the page. How do I get this to come up.

Ive only just come accross this post, did you ever manage to get it setup? Are you now using version 3? If you need any help please let me know or any feedback
