DPA Questionnaire for Cloud Services

I'm looking at doing a questionnaire for Cloud based providers who we are looking into buying their services, the idea is to make certain checks to show the ICO we are actively trying to protect our data.

 

Is anyone else doing this sort of thing and have any examples of questions asked?
