Its easy to make a copy of your form for public viewing

I'm not going to post up mine in case I get a load of fake jobs created but mine is really basic, Class, equipment type and description.

I really don't want to give them too many boxes to get confused with.

 

edit - mine also has drop down boxes for class and equipment type

You should increase the font size , what's to stop the acknowledged by technician being ticked by someone else ?

That's the point of this thread - you can create a copy of your form and post it on here without worrying about people messing

Ok so Teacher fills in the top bit of the form.

What happens to the progress and compleated bit.. Does that get seen by the teacher again or is that just for your internal team?

The "progress" bit is only seen by you.

 

All the captured data is stored in a spreadsheet. When completed I perform a 'strike-through' through each row that I've completed.

We do different forms for different areas.

 

Here is one we do for a purely google environment:-

We normally set it to collect the user email information from being logged into their school google account.

We have set them to automatically email a notification to a helpdesk mailbox when ever a user posts a log

Entirely up to you

I put a link to the spreadsheet so that anyone can see all the past issues and their progress
