Simple PHP script for notes about students?

I've an idea that a PHP script that allows any teacher to make (and add to) notes about pupils would be very useful in my school. (Web front end, PHP, MySQL)

 

I was wondering if anyone was aware of a PHP script that does this or has that feature included.

THis should be secured in SIMS.net or CMIS or similar school management system. Unless you want to be fully responsible for the data protection issues?
