Moodle quiz page issues

Hi all,

 

Trying to add a quiz on Moodle which works ok when adding a new question, but when I go to add a second question I get a 500 error.

This doesn't happen if I go to Update Quiz after every question I add which is a bit of a pain to do for every question.

 

When the 500 error appears, I noticed that the URL is odd looking (not knowing what it should look like though).

The bit that sticks out the most to me is "http%3A%2F%2F"?
