IEP Writer Format issue

We have IEP writer version 3 installed as a local application on a networked machine, with no access to SIMS or anything.

It is working OK but for one glitch: having inputted the names etc as requested so that the surname shows on the complete form but the first name in the text, when a record is reopened sometimes the first name has been replaced by the surname.

I can't find anything in the manual and the weird thing is: the program is installed on 4 other machines and only one of them has this problem

 

Anyone come across this or can point me in the direction of help?
