Data Protection Impact Assessment - does anyone have a template they could share?

Hello all,

 

I need to get cracking with a Data Protection Impact Assessment for our school's data and am going to create a form that will cover the sensitive data we have at the moment and that can be used in the future. I was just wondering if anyone might have a template they are using that they would be happy to share?

 

If not, I'll knock one up and share it here!

Our DPO Service sent us this https://ico.org.uk/media/2258857/dpia-template-v1.docx straight from the ICO website https://ico.org.uk/for-organisations/guide-to-the-general-data-protection-regulation-gdpr/accountability-and-governance/data-protection-impact-assessments/

 

Explained that if the ICO ever want to see them it will be in there format and easy for them to pick through.
