Creating User Input for Achievement Certificates

The charity organisation I volunteer at have asked me to create an achievement certificate to hand out for special achievements or occasions, including an official marriage certificate for marriage ceremonies we perform. I created a template in Word, but I would love to be able to have a document that automatically prompts the user for inputs and populate the data in the certificate from there. I would also like it to automatically generate a document number so that each certificate is unique. Is there anyway I can achieve this? I have tried content control boxes and {FILLIN} commands, but I can't seem to get this working the way I want it to. Also, I have no idea how to generate the incremental certificate number.

 

Any ideas? Am I doing this totally wrong? Would there be a better way to go about it?
