sending a form via microsoft outlook 2013

Hi everyone, I've had this problem for a while and I've been trying to figure this problem via Google and other forums. I've created a word document that is a form and I would like to send this to contacts to be able to fill in the e-mail itself and not have to open word directly and complete by that. I need them to be able to open the e-mail and the form to be there so they can complete and then be able to send it back to me. I don't know if this is completely possible to do - but any feedback or guidance would be appreciated. Thanks, Jack.
