Does anyone have a 'contract' for loaning equipment that I can borrow and tweak?

Not particularly formal, but it is sufficently generic and covers all eventualities.

 

All items are bookable for a maximum of 7 consecutive days, unless previously arranged. A minimum of two full working days notice is required for all bookings. Check condition and report any omissions / visible damage before signing out. All faults/damage occurring during use must be reported in writing on return. Certain items are issued as complete kits and are not to be split up or swapped around. Everything must be returned clean, on time and in the condition in which it was received. Failure to comply with these conditions will result in loss of privilege of access. Every item lost or damaged through negligence will be charged for at current retail cost. If you book equipment, you are responsible for it; do not book kit for others!
