RTF Reports in SIMS

Hi I was wondering if anyone could possibly help me please. I am trying to create my own ERTF report in sims, Bascially I want a table showing me

 

STudent name

Address

Parents contact details

Class

Medical conditions

Medical Summary

Medical condition ID

 

Whichever way I produce this report the problems I seem to have is that the for example one pupils medical condition may show up 5-6 times as well as their name tec, I have ticked/untciked the surpressed duplicates box but this still doesn't do much. Is their anyone that could possibly help me please
