My Mac seems to think it has 2 instances of OSX, Pictured is the Boot Options screen, the instance on the left works find, and the one on the right produces the susequent error.

 

I've restored the whole machine from TC but the problem comes back.

 

It doesn't affect the functionality of the working OSX instance, it's more of a personal OCD issue of it being there and not functioning.

Have you tried doing a chkdsk on the one that doesn't work maybe it could give you another clue?

Does the over disk work when booting in to safe mode (if it's there at all in safe mode)?

That error normally means a part of the system is corrupted or has been moved / broken. If the partition is of no use to you, you can delete it using Disk Utility, reclaiming the unused space (be aware that this will delete any data stored on that volume). If you need it to boot, you will ned to start up from the recovery partition and re-install OS X on that volume.

There's only 1 partition in disk utility.

Have you tried resetting the PRAM (hold down cmd + alt + P + R on startup until you hear the startup chime 4-5 times)?

 

Failing that I would try taking a disk image of the working partition, re-partition the whole drive and restore from backup. Quite a bit of work and not guaranteed to fix it so might not be worth doing.

Have you got a RAID set up with two hard drives installed? The only time I've seen that is with RAID. We have a mirrored RAID set up on our Apple Server in our environment. When we boot into the boot option two HDs appear titled Macintosh HD because there are two drives. Then in Disk Utility in the OS the RAID only comes up with 1 partition.

 

When one hard drive died (the one set to boot first) we got the no entry sign. The second drive booted fine. We replaced the first drive, re did the RAID in Disk Utility in Recovery mode and restored the OS from TimeMachine.
