Digital Camera Loan Form

Hi all,

I have been tasked with creating a loan form for classroom based digital cameras. I have an outline but I feel like I am missing something.

 

Anyone got any pointers or little bits that they could help me with so that I can get this setup and in place with it being as nearly airtight as possible?

 

Any help will be greatly appreciated however.

p.s. in the near future I will be also doing one for loaned laptops. Some help may be needed with that too.
