I have the same Problem with a Dell PowerEdge 530. I think, this error may occur, when there is heavy load on the disk (-array), where the hypervisor resides. e.g. if a guest VM on the same disk (-array) downloads and installs windows updates.... . I went back to 2012 R2 and have no problems anymore. Can you verify this relation between the crash and the load on the disk (-array)?
