not sure you can do that, usually forms already come as pdf that you can write in :/ converting pdf to word is really complicated, ive used one before (think it was Create Word document | PDFtoWord Pro ) but that only converted the text and no images and it was all a bit jumbled up... real software costs quite a bit of money, and im not sure anyone is that keen
