Staff training questionnaires

I want to get a picture of how (un)-able the staf here are to perform basic tasks. I was thinking of sending out a questionnaire asking what training they may require, Office, IWB, VLE etc.

 

Has anyone got a questionnaire I could modify for my purposes (basically im being lazy and dont want to reinvent the wheel)
