Self Survey/Questionnaire

Hi,

 

Just wondering if anybody has got a good self survey/questionnaire for the ICT Tech Support Department that I could have a look at. I'm thinking of asking HOD's and SLT to fill them in so I can get a better idea on how we are doing.

I did this at my last school, after 3 months I did a questionaire too all staff and asked them to rate the service now etc etc
