School licenced software on teachers home computers

Does anybody have a document for teachers to sign when they install school licensed software on their home computers that I could plagiarise?

 

Software like Adobe CS4 has a School Site License that comes with an additional 50 teacher home-use rights licenses.

 

Something that put the emphasis on the teacher to remove the software when they leave the employment of the school, and wavers the school from ensuring that they do
