Has anyone created a custom Sharepoint site?

We use Sharepoint heavily and now want to look at creating a custom design so w can more easily access all the sites we have.

 

Has anyone created a custom Sharepoint design and if so how?

Before we ditched SP in favour of something more usable (Joomla!), I did. It was mostly a bunch of CEWP. I used Sharepoint Designer for basic layout. It wasn't too difficult but our site was very basic.
