[ms office - 2010] Outlook Survey form works for me but not others?

OK, Here is the deal.

 

I designed an outlook form, which allows for a survey to be made in outlook. I can make the survey, and can send it too myself, and it works perfectly fine. But whenever I send it to someone else, it comes up as code and I am not sure why.

 

I am using Outlook 2010, and I designed the form in the design viewer panel. Image attached showing the form.
