Barcode Maker for Library

I'm after a program that will make bar codes to print off on labels.

 

I want the bar-code to be a number.

 

also i would like to add text top and bottom after the barcode.

 

Any ideas?

Do you need this to integrate with a library database program or similar?

If not, just download the free 3 of 9 barcode font and use excel.
