Can someone send me a student admissions/application form?

Hi all,

rather than reinvent the wheel I thought I may ask here...

We use Sims and I wanted to update the schools student application form (the one parents complete when the student wants to come to the school)

Ideally I want one that follows all the sims fields as the one I have doesn't!

so basically it would go:

1: Basic Details

2:Registration

3: Address

etc etc...

 

let me know cheers or it will mean I have to start building one now
