Looks like it can't connect to the MySQL database that it expects to be running on that server. Obvious things to check:

    • MySQL is installed
    • MySQL is running
    • The appropriate username and password has been created
    • The OPT database exists

The necessary details are probably in the C:\inetpub\wwwroot\opt\opt\config.php file for you to check.

Looks like your MySQL database is either not up and running or the login details are incorrect and the PHP script can't access it.
