    Thanks so much for flagging this - this resolved our problem with "an error has occurred" as well.
    I noticed there is a new (modified) option within Lightspeed's Rule Sets: "Block Google and Youtube HTTPS (fail-safe)". Previously this option was just for Google HTTPS. This new setting caused an issue for the latest YouTube iPad app as it now uses https: to the youtube API.

    It occurs if the High Definition Audio driver is being used and not the vendor driver for the sound card. Update your sound card drivers.

    Had the same problem occur today on 1 machine out of 4 identical all with the Windows HD Audio Driver, updated driver and it still didn't work, plugged in a pair of headphones and it worked, how daft is that.

    Had the same issue and enabling ActiveX filtering resolved it.

    Unfortunately for you, at the school where I work, headphones/speakers were the fix to this particular problem. I'm still looking for other solutions that don't involve older versions of Firefox and/or Flash.

    Update: I fixed the problem permanently by installing the actual sound card drivers instead of using the Microsoft High Definition Audio drivers. I have a Dell so I just went to their site, put in the service tag, showed all drivers, downloaded the SoundMax Integrated etc..., extracted with 7-zip to a directory and updated the drivers manually. Works perfectly in Firefox and IE now (never stopped working in Chrome).

    To deploy the driver to all your dells you would need to script it or Copy the driver files to the driver store or use pnputil. If your lucky and have sccm you can jusr push out a driver package.

    Morning all

    I seem to have same issue...

    all PC are getting black fuzzy screen with an error has occured....

    I have speakers attached to my PC and still no video..

    strangely it works for Chrome now but not IE8

    do i need to upgrade to IE 9?

    Hi, Same issue here. We too are using Lightspeed Filtering and it seemed to stop working after updating to version 2.6 There are two machines that bypass the filtering system and these seem to be fine. I have tried the normal steps of updating sound drivers, clearing cache etc to no avail. Google Chrome was a fix yesterday. Today its not working in Chrome, so at present I have no fix. Any ideas?

    We've had the exact same issue, plugging in speakers or headphones has sorted it straight off. Really weird that there's an audio requirement somewhere!

    We have been having the same issue for a couple of weeks now. It was first of all limited to just some teaching staff, but it has now spread to the majority of the school. I have contacted CAPITA and they have said that they are aware of this issue as other schools are having it as well.

    We had the same issue, it started with a few clients then spread to the whole network.
    We have IE10 with auto update set, latest versions of Java & flash.
    The fix we have used is to set Googlevideo as an allowed keyword in our rioters (NetSweeper)
    I hope this will help some one else as it was a right pain in the ...
