Permissions looks about right but have you checked the subfolders? you could try 'Replace all child object permissions with inheritable permissions from this object' to force all subdirectories and files to have the same permissions, then test.

How would it? The start menu contains a few hundred kilobytes of shortcuts. Copying them on login would take seconds on any network, even a slow one! Microsoft's guidance is that you should enable offline files when using redirected folders, else you're risking things disappearing randomly.
