    forms

this isn't strictly a webdev question. we recently published our Options booklet, and I was asked to cut 'n' paste the returned info into the school website from a word doc. staff had been given/emailed a word doc with a table phrases such as,
"Why [your subject] is important", and as you can guess, when the booklet went out, (I know it should have been proof read beforehand, but that's another matter), a lot of faculty heads hadn't replaced the "{your subject}" section with their subject.
It struck me that this could be done properly by having staff fill in an online form which returns an error if there is incorrect or missing information. I want the form not only to do the job, but to make life as straightforward as possible for the repro dept. and myself. TIA

    Re: forms

There are php-based form generators on Sourceforge - some that will submit to a mysql database and some that email results to a defined email address.
