Web survey database
I have been asked to look at writing some webpages that ask the pupils some questions and the answers are recored in a database that is easy but what we want is pupils not to be able to answer the questions more than once.
I need the asp page to get there username from network and put that into database and then if thay try again it will check username against database and see if that have answered the questions before.
Any one help or point me towards a site that has asp code that will do what i want

If you've got access to a MySQL DB and PHP I can have a dig around for a web application I wrote a couple of years ago that allows you to create questionnaires which should fit your needs. Its a little basic in some ways but does the job.

I do not want to buy software to do the job. As this will only be a one off.
YEs i have access mysql and php so if you have an app that will do the job great
