It is possible but might depend on your skills! I'd use ODBC to connect a powershell script to the moodle database and process the users table, looking to see if users already existed and creating them if not. This might get more complex depending on how your organise your users into AD's Organisational Units. This script would probably have to be scheduled (run overnight or every 10 mins or whatever) - although that might depend on your mysql version, it should be possible to use a trigger on the users table to kick the process into action but that's probably quite difficult for reasonably little benefit.

Moodle 2.0 onwards has this as an option. In the LDAP configuration page select 'create user externally' in the 'enable user creation' section. I've never used it though so no idea how well it works or if it plays nicely with AD

Why not do it the other way round? Create the user in AD and then let Moodle log on using LDAP as authentication.

right, we use the facility to moodle block to add users to moodle, having moodle then create the users in the AD would save so much time.

The Fac-2-Moodle block should only create users if it detects that they exist in AD with the same ID Number. AD users should be created first before running the Fac-2-Moodle import. Generally via a CSV export of users from Facility bulk-imported into AD... THEN run Fac-2-Moodle.
