Display Screen Risk Assessment Ticksheet Request

Does anybody have a ticksheet for performing display screen/office space risk assessments?

I have the HSE guidance and will be working with the bod in charge of H&S here to create one... we are a little lazy though and would like to knick somebody elses (with their permission of course ).

Well... I created my own. It isn't a comprehensive form but it covers the basics.

It is designed to cover the key points from the HSE guidance and, as such, should be used alongside this.

The form is designed so that every 'No' tick should have an action associated with it.
