As someone said above, use Google Docs, click new survey, all done very quickly and nicely. Results collected in a Google DOcs spreadsheet that can be exported to excel if you choose. About a 5 minute learning curve, and all free.

Another vote for LimeSurvey
Installed, configured and Survey created in very little time (and that's with the LEA blocking the installation guide websites!!)
