    Form created with Publisher

I know it's a lousy way of doing things but I said I would try and get an on-line form set up before OFSTED come in on Wednesday. I've created it using Publisher for speed but the SUBMIT button whch is supposed to email the details boms out with a WEBBOT-SELF message. I suspect this is because our LEA broadband doesn't support Frontpage extensions.

Now I could design a form using Dreamweaver which may or may not work but I've never done it before and I would rather be enjoying the sun than sitting at my computer all day.

    Has anyone any ideas?

I suppose I could find someone to host it for me until I've found a better solution

if you are looking for quick and easy ones to setup (but with some limitations due to being free services) i would recommend either http://www.surveymonkey.com or http://www.formsite.com

neither are useful for longterm solutions really (probably surveymonkey would do if you pony up for a paid membership) but they do in a pinch

You can make quick and easy froms in google docs. Submissions collated to a spreadsheet, all done very nicely.

You can also use the open source Lime Survey project... I have it installed @ LimeSurvey (but don't use it...), Should be a snatch to install/setup if your webhost has a automatic install solution (IE Fantistico or installtron)
