Is it on a dell? ATI make cards like this. I think they are called DOV cards

...yeah, it's a Dell - I've just discovered I can find it by Googling 59 pin DVI.
Its a DMS-59. You can get them from most suppliers.

I've also seen those as an input dongle on sanyo projectors...

We bought a few of those by accident thanks to the crappy terms in use. They're used by the old majority of multi-head graphics cards. It's annoyingly difficult to find DVI to DVI & VGA splitters from anywhere except RM, who've got a stupid delivery delay!
DMS-59 isn't that common on regular desktop graphics cards, mainly on workstation/professional cards such as nVidia Quadro and AMD FirePro cards.
