Feedback: I wasn't sure if the "How many virtual servers do you have deployed at the moment?" question was for number of physical servers used for virtualisation, or number of virtual servers spread across hosts. In my case the answer is the same regardless, but just thought it could be more clear

You missed out common problems/decisions with virtualisation projects in your survey (vm-sprawl: depts who'd normally give "x" out of their budgets for a physical server thinking virtual servers are "free", backend storage decisions, vendors not supporting their products in virtual environments)
