This is ours:
Use of Laptop Provided by School
All staff issued with a school laptop need to read the following information.
The laptop can be used in school and at home. Therefore you may remove the laptop from the school premises and use it for both school work and personal use. The use of the laptop is restricted to members of the school population only. Other staff members or school pupils can use the laptop when plugged into the network system but they must use their own username and password.
You will be given a local username and password so that the laptop may be used without being connected to the school network. Do not allow other people to use your laptop with this username and password.
When using the laptop off the network, using the local username, you may install any software that you own as long as you own the license for this software. Please be aware that you may be asked to produce evidence by the school to show that you own the software that is installed by you.
Requirements to keeping a laptop:
a). Plug your laptop into the school system and log on to the school system at least once per month. This ensures that your laptop will synchronize with the school server.
(This keeps the virus software up to date and keeps the laptop free of viruses).
If you do manage to infect the laptop with a virus, return to system support immediately.
b). If the laptop is damaged in any way, return the laptop and report the damage immediately to systems support. A damage report will need to be completed by yourself and you will have to have it signed
by a member of the SLT. Do not attempt to fix the laptop yourself as this will invalidate any warranties. (You may be charged for damage to a laptop depending on circumstances).
c). It is your responsibility to ensure that the laptop is kept in a safe place at all times to prevent anybody from stealing it and to stop it from becoming damaged. If your laptop is stolen outside of school, you will need to provide evidence, e.g. a police report and you may need to claim on your house contents insurance.
d). Occasionally we will recall the laptops for upgrade. You will be informed of this by messages over the school network. You will need to do this immediately to ensure the laptop is kept as up to date as possible.
e). If you should become ill or are away from school on non school related business for a matter of 15 school
days, you will be required to return the laptop to systems support. This does not include school holidays.
f). Please keep all items of your laptop, eg power supply, mouse etc in the bag provided as lost items will be charged for.
g). If you do not feel that you are able to maintain the above rules, please return your laptop for redistribution.

I've attached the one we use for you, I think I went trawling the 'net and cobbled together a couple until the head was happy with it.

Staff login to the same domain when logging inwith school, but the work they do using a local account at home is not backed up.
"...... Staff members may use the school’s laptops for limited personal purposes ...."
but be aware that if used for personal purposes it then becomes a taxable perk and must be declared to the IR !!!!
